Program Release Form

Please complete this form if you will be arriving/departing early or late from a program. It is important to provide accurate and thorough information. Please note that our programs start and end officially at the gateway city airport (ie. Madrid, Paris.) We will contact you to let you know any additional escorting fees. Generally, we charge $150 + travel expenses if a student needs special escorting outside our normal pick-up/drop off times. Thank you!

    I (Parent/Guardian listed on the online application) hereby release the above named student from the SPI Study Abroad, Inc. "SPI" Program as listed above on the designated dates and times. I understand that SPI may used third party providers to assist students arrive at the point of departure or arrival. SPI has taken all prudent care to provide students witha safe and efficient manner in which to get them to the site where they will be officially released from the program. Students may be traveling for portions of their travel alone or unsupervised.
    I ACCEPT THE RISKS ASSOCIATED WITH STUDENT LEAVING THE PROGRAM EARLY OR ARRIVING LATE, AND I HOLD HARMLESS SPI AND ANY OF ITS DIRECTORS FROM ANY DAMAGES THAT MAY BE INCURRED FROM THIS RELEASE.
